A Guide to Our Services

CLACKS WELCOME POSTER.jpg

Registering with our Practice

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from reception. You will be registered with the Practice rather than an individual GP. You will also be given a questionnaire to complete detailing your medical history. You will be given an appointment with the Practice Nurse before being accepted as a patient. Please bring your questionnaire and a sample of urine with you to this appointment. Identification is required to register as a new patient, e.g. a passport, driving license or utility bill.

Please note, if any details of the registration form are incomplete you will not be registered. Remember if you change your name, address or telephone numbers, please inform Reception, so that your details are always up to date.

Your Local Pharmacists

Your local pharmacist will be able to give you free health advice at any time - you don't need an appointment. They can advise on minor ailments and give treatment if necessary; they will also issue you with a short supply of your repeat medication at the weekend, should you run out, until your surgery reopens.

We run a range of clinics. For an appointment or further details, please call the Practice on 01324 822 330.
